School Nurse Mini Grants

The Utah Asthma Program will award up to 10 grants of $500 to school nurses to complete asthma-related projects. Priority will be given to 10 qualified applications received that are complete and meet the minimum requirements outlined in the project description. Grants for the 2024-2025 school year will be posted in September 2024.
How to Apply
Step 1: Confirm Eligibility
The Asthma Mini Grant program provides grant money for school nurses in the state of Utah. To be eligible, the school nurse must be a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Nurse Practitioner (NP) who is employed or contracted by a school, school district, or local public health department to provide school nursing services.
Step 2: Complete the Application Form
Funding priority will be given to 10 applications received that are complete. A form signed by a school administrator is required as part of the application. Please email applications to jljoseph@utah.gov.
Step 3: Await Notification
You will receive a confirmation email that we received your application within 2 business days of submission. If you do not hear from the Utah Asthma Program within that time period, please contact us to confirm that we have received your application materials.
Step 4: Complete the Project
For applicants that are awarded a mini grant, the project should be completed by May 2025. The Utah Asthma Program will provide you with a packet of materials and resources related to the project you choose. Jesse Joseph (jljoseph@utah.gov) will be available to offer support and answer questions as needed.
Step 5: Return Outcomes Forms
After the project is complete, you will need to submit an outcomes report. Outcome reports will be due no later than May 17, 2025. Payments will be processed after the outcomes report is submitted showing completion of the project.
